Standard Workshop:
Come join us on South Beach at the Hilton Bentley Hotel. Valet parking available at a discount price
of $8.00 for workshop participants. Projected itinerary below for South Florida workshops ONLY.

To participate download the "General Registration Form" above and mail form with check made out
to (5FAB Ministry) to the address stipulated on the form as soon as possible. PayPal service also
available below. Prices are as follows: $75.00 per person ($50.00 if organization / ministry sends
three or more individuals at a time by the date (?) Senior Pastor (only) can attend for free with any
paid participants of their organization / ministry attending these workshop, to include any department
head that has already previously paid for a previous workshop. There will be a $25.00 increase per
person following the deadlines stipulated above ($100.00 per person or $75.00 for every three
individuals from the same organization / ministry.) Workshop limited to 30 individuals.
